Case Manager CONTACT NOTES

The following occurred:
·         Received a call from staff telling me about their plans, he also asked about the calendar and when we were planning on meeting on Tuesday.
·         Email John and staff the current weeks schedule.

Date2/15/11                Type of Interaction: Calls/Meeting/Email/research L.O.I: 2 ½ hours
The following occurred:
·         Called Spectrum about John possibly volunteer there. Was given a different number with no answer.
·         Met with John and staff, talked about how everything is going. Tried some of the bread John made and we discussed how John and staff are going to plant vegetables. If the plants grow John is interested in making some of his salsa to sell at the farmers market. 
·         Emailed guardian to let her know that there were documents I needed her to sign at John’s.
·         Searched possible volunteer jobs that John may be interested in. Will speak to him about them on our meeting on Thursday. Emailed the ideas to my boss to get her opinion on the different locations.

The following occurred:
·         Called staff to verify time to go bowling tomorrow. He said they were currently at John’s PT appointment and John was in the pool today. Staff said that 12:30 would work best for bowling tomorrow.
·         Called the bowling alley in South Burlington to make sure they would have room for us to go bowling they stated they did.

The following occurred:
·         Received a call from staff and John to verify time to meet at the bowling alley and location.
·         Meet them at the bowling alley and enjoyed a great game. John in showing great gains with the new ability to walk without his can while carrying a bowling ball then actually throw the ball and get a strike. Today was a positive day for John and he was very gracious. 
·         The staff mentioned that John is interested in going to a batting cage to see what he can do. I told him I would look into local places that offered them.
·         Looked into local batting cages and emailed staff the location. But before they go I want to make sure this is something that is safe and he can physical do.
·         John signed his February team meeting note. We discussed tomorrows plan for the Voc Rehab appointment and the time I would pick him up.

Date2/18/11                Type of Interaction: Calls/Emails/Weekly Documents     L.O.I: 3 hours
The following occurred:
·         Called staff to make sure we discussed the batting cage at the next team meeting to ensure the guardian was ok with it. Staff agreed and told me this was something they had thought of because they use an old mop handle and a ball. John bats at the balls staff tosses.
·         Sent a follow up email to my supervisors about the batting cages to make that way to proceed with them.
·         Updated John’s EFS and sent the team a current copy. A long with next week’s calendar.
·         Met John at his apartment and took him to his Voc Rehab appointment. At the appointment we met with a lady from the Barre office along with his voc rehab counselor, and her assistant in Burlington. They asked John some routine questions about what he would like to do for work and what skills he could bring in. She set up a follow up appointment to give John a cognitive assessment at his apartment for the following Friday at 10:30. After the assessment we will have a better understanding at what level John’s cognitive skills are at. She did let me know that this cognitive assessment would not take as long as one say from doctor Smith. But as far as she could see in John’s records he has never had one. John was very excited about the outcome of the appointment. We were also able to get her to fix the voucher from John to get free bus passes. 
·         Completed weekly paperwork.

  1. Alysha, thanks for posting some examples of contact notes. A file will have many pieces and they all require the attention of being detailed and organized.
